Mitochondrial genome maintenance exonuclease 1 (MGME1) is a critical protein involved in the maintenance of mitochondrial DNA (mtDNA) integrity. This protein belongs to the family of DNA exonucleases, which are enzymes that catalyze the removal of nucleotides from the end of a DNA strand. MGME1 specifically possesses a 5'-3' exonuclease activity, which is essential for the processing of DNA during replication and repair within mitochondria.MGME1's primary function is to participate in the mitochondrial base excision repair (BER) pathway, a crucial mechanism that corrects oxidative DNA damage incurred by the reactive oxygen species (ROS) that are produced as byproducts of oxidative phosphorylation. The enzyme ensures the proper processing of mtDNA flap structures that can arise during the replication of the mitochondrial genome or as intermediates of the BER pathway. By doing so, MGME1 helps maintain the proper replication of the mitochondrial genome and thus ensures the fidelity of mtDNA.

Mutations in the gene encoding MGME1 can lead to mitochondrial dysfunction due to the accumulation of deletions and other mtDNA abnormalities. This can result in a spectrum of mitochondrial diseases, often characterized by progressive external ophthalmoplegia (PEO), muscle weakness, and multisystemic disorders, reflecting the essential role of MGME1 in energy-producing cells.MGME1 is also considered to have a role in the mitochondrial DNA replication process itself, facilitating the turnover and recycling of mtDNA replication products. This is crucial because the mitochondrial genome encodes for several key components of the respiratory chain complexes, which are indispensable for ATP synthesis.
