21-Hydroxypregnenolone, a steroid hormone, assumes a pivotal role in the biosynthesis of glucocorticoids and mineralocorticoids within the adrenal gland. Its synthesis occurs through a series of enzymatic reactions, starting from cholesterol, with regulation mediated by the hypothalamic-pituitary-adrenal (HPA) axis. While the precise mechanism of action of 21-hydroxypregnenolone remains incompletely understood, it is believed to exert its effects through multiple targets within the brain, including GABA-A receptors, NMDA receptors, and sigma-1 receptors. As a positive allosteric modulator of GABA-A receptors, it enhances the inhibitory effects of GABA on neuronal activity. Simultaneously, as a negative allosteric modulator of NMDA receptors, it diminishes the excitatory effects of glutamate on neuronal activity. Furthermore, 21-hydroxypregnenolone binds to sigma-1 receptors, which play roles in diverse cellular processes, including neuroprotection and neuroplasticity.
