2-Hydroxy-3-methylbutyric acid is an intermediate in the microbial metabolism of hydrogen fluoride and caproic acid. It has diagnostic value for identifying probiotic bacteria. This acid has been observed to induce cell lysis, potentially due to its ability to enhance polymerase chain reaction (PCR). It is utilized as an internal standard for the analysis of ethylene glycol (EG) and γ-hydroxybutyrate (GHB) in whole blood. Additionally, 2-Hydroxy-3-methylbutyric Acid is used as a marker in chemical diagnosis of organic aciduria and other inborn errors. It is also used as a metabolomic biomarker in preeclampsia.
