Capecitabine may enzymatically converted to 5-fluorouracil (5-FU) in tumor cells. 5-FU is an antimetabolite that inhibits thymidylate synthase, leading to the depletion of thymidine triphosphate and the incorporation of fluorinated nucleotides into DNA and RNA. This disrupts DNA synthesis and repair, ultimately leading to cell death. 5-FU is incorporated into RNA, leading to the inhibition of protein synthesis. Capecitabine′s conversion to 5-FU occurs preferentially in tumor cells due to higher levels of thymidine phosphorylase, an enzyme involved in the conversion process.
