Benzonatate, a synthetic butylamino-benzoate derivative, acts as a peripherally acting antitussive. It functions as a reversible blocker of voltage-gated sodium channels, specifically inhibiting Nav1.7 currents in a concentration- and voltage-dependent manner. Furthermore, it suppresses action potential firing in catecholamine A differentiated (CAD) cells. Structurally resembling polyglycol ester local anesthetics like procaine and tetracaine, benzonatate is classified as a butylamine. Upon hydrolysis by plasma butyrylcholinesterase (BChE), it yields the metabolite 4-(butylamino)benzoic acid (BABA) as well as polyethylene glycol monomethyl esters.
