遊離酸である(+)-15(R)-フルプロステノールは、強力なFP受容体アゴニストである2。ほとんどのプロスタグランジンの15(R)-エピマーは、天然の15(S)-異性体よりも約100倍作用が弱い。しかし、フルプロステノールシリーズでは、C-16に隣接するフェノキシ基がC-15の絶対配置を逆転させる。したがって、15(S)-フルプロステノールは15(R)-フルプロステノールよりもFPアゴニストとしての活性が低い。
