プロスタサイクリン（PGI2）の第二世代の構造アナログで、カルバプロスタサイクリンに代表される第一世代の安定アナログの約10倍の効力を持つ。 イロプロストのほとんどの製剤には16(S)と16(R)の立体異性体が含まれている。また、血小板凝集を阻害し、そのIC50値は65 nMである3。
