Fenofibric acid functions as a peroxisome proliferator-activated receptor alpha (PPARα) agonist. It acts by binding to and activating PPARα, a nuclear receptor that regulates the expression of genes involved in lipid metabolism. Upon activation, PPARα forms a heterodimer with the retinoid X receptor (RXR) and binds to specific DNA sequences known as PPAR response elements (PPREs) in the promoter regions of target genes. This binding leads to the transcriptional activation of genes involved in fatty acid oxidation, lipoprotein metabolism, and glucose homeostasis. By modulating the expression of these genes, fenofibric acid ultimately influences lipid and lipoprotein metabolism, leading to changes in plasma lipid levels. Its mechanism of action involves the regulation of various enzymes and transport proteins involved in lipid metabolism, such as lipoprotein lipase, apolipoprotein A-I, and apolipoprotein C-III.
